Community access to services

In 2024 City of Casey identified through community research that some people were having trouble accessing health and support services. We are therefore undertaking additional research to help us understand what Council can do to make services more accessible.

This research is completely anonymous and the survey will take between 5 and 15 minutes to complete, depending on how much detail you wish to provide in your answers. You can save your progress at any time by using the 'resume later' link at the top right of the screen. This will email you a link to re-enter your survey - we cannot see your email when you use this functionality.

All questions have a ‘prefer not to say’ or ‘don’t know’ option at the end of the answer list that you can choose if you don’t feel comfortable answering.

Collection statement
The personal information requested on this form is being collected by Council for the purposes of understanding barriers to service access. Data will be collected using the survey platform Lime Survey and managed by third party contractor ASDF Research, who will assist with the collection, analysis and reporting of the data. ASDF will provide Casey Council with the data files and a summary report containing de-identified information. The deidentified report will be published and made available to the community. By completing this survey you confirm that you understand that if you provide my personal information as a part of this review, that this will be collected by a third-party consultant and provided to Casey for the purposes of publishing a report for the community. Personal information will not be published in a summary report, but verbatim responses may be. If you wish to gain access to the information provided or amend any of the personal or business information you have supplied to Council, please contact Council via telephone on (03) 9705 5200. Council may not be able to amend anonymous responses if submitted anonymously. Council will not be liable for copy or misuse of the information contained in this form by third parties.
